These new clinical pathways follow the direction of the eight pathways that HealthPRO Heritage at Home originally developed for PDGM. It starts with pre-admission, admission, treatment, early benchmarks, late benchmarks, and discharge planning. Now our clinicians and partners have a total of 20 pathways specifically focused on skilled home health utilization.
