Twelve New PDGM Home Health Clinical Pathways

Earlier this year in quarter one, the National Association of Home Care and Hospice (NAHC) released preliminary data specific to the early PDGM claims and trends*. This was our first opportunity to be able to begin benchmarking performance compared to the trends that were occurring across the home health industry. the average and most consistent top five (5) diagnosis codes submitted in each of those categories. Our home health leaders have developed 12 new clinical pathways that create opportunities in which we can improve clinical care, treatment delivery, and overall quality outcomes for our patients.

These 12 new PDGM clinical pathways include the following:

  1. COVID-19 [Coronavirus Disease 2019]
  2. CVA [Cerebrovascular Accident/Stroke]
  3. Dementia
  4. Diabetes
  5. Falls
  6. Spine
  7. Lower Extremity Fracture/Surgery
  8. Upper Extremity Fracture/Surgery
  9. Lymphedema
  10. Multiple Sclerosis
  11. Parkinson’s Disease
  12. Genitourinary

These new clinical pathways follow the direction of the eight pathways that HealthPRO Heritage at Home originally developed for PDGM. It starts with pre-admission, admission, treatment, early benchmarks, late benchmarks, and discharge planning. Now our clinicians and partners have a total of 20 pathways specifically focused on skilled home health utilization. HealthPRO Heritage at Home ensures that these pathways will be successful clinical tools in guiding clinicians on evidence-based tests and measures, functional treatment options, educational literacy, and proper expedition through the continuum of care.
