- Curate activities tailored to all dimensions of wellness, to enhance mental and physical well-being, support holistic care, and foster successful aging.
- Collaborate with interdisciplinary teams to leverage participation data to address patient needs, improve care planning, and strengthen communication with families
- Provide onsite program assessments, departmental analysis, and ongoing services, including program development, training, and dementia-focused strategies.
- Remote MDS completion by expert nurses; bringing accuracy, efficiency, and confidence to your SNF/ LTC units and swing beds.
